My thoughts on this scene:
I'd been worried that the premier would involve a medical mystery. I thought it'd be ruined by House running around saying "she's not crazy, it's just a...thyroid condition. or lupus?". So what I loved about this scene was that it was really the only moment where someone consulted him in his capacity as a doctor. And it wasn't for a mystery. It was for something completely mundane, and completely heartbreaking. So much of House's sense of selfworth is tied to his job, so when he saw that the consult could have been done by any other doctor, his instinct was to exercise that other part of his talent that he prides himself for - figuring people out. I think the fact that Nolan seemed to be alone in this horrible moment, despite how stable he comes off as, and thought that maybe House could be of some use in that situation, helped House make that step to just sit down, shut up, and wait with the man. Maybe the dying father aspect played into it, maybe not. Still, I thought it was a good choice, that scene.
