The great thing that's been mentioned before by others here is that if House does indeed betray Wilson's trust in this way, by saving his life when Wilson wants to either die quickly or cure the thing once and for all, it would be as though House had taken on Stacy's role, back when he had the infarction: going against someone's will to save their lives, because he loves them. Wilson is doing something incredibly Houseian--nearly the same thing House did with his leg, risking death to get a shot at a "best case scenario" even though it was likely going to take his life--and for once, House is the spectator. If House does do this, I imagine there will be some amount of tension between them, to a greater or lesser extent, in the following episodes.

The road trip seems a little bit more amiable than I expected, but think of the title "Post Mortem". It's likely or at least possible the road trip serves in part as time for House and Wilson to go over what happened in the episode prior, as a post mortem can mean many things, one of which is the discussion of something that has come to pass (like what we do here after an episode has aired). It seems likely the episode title has a double meaning, as many House episode titles do.

A lot remains to be seen, and this is just a theory, though.
