That scene really brought home to me how much this show has changed, and how much the audience has changed. In S1 we would have thought that a doctor telling a man's date that he's a compulsive nosepicker was outrageous. Now not only does the "nice guy" doctor do it, and the "nice girl" doctor thinks nothing of it, but we don't react either.
