If you ask me, it seems more likely that Wilson tries this risky treatment in episode 19 (and we know it is the treatment killing him, not the cancer), but in order to save his life, House intervenes somehow; probably takes him to the hospital against his wishes. Many people say they can't see House doing this but I sure can. Once House stops the treatment and saves his life, Wilson would eventually--I'd say quite rapidly--return to "normal", but still with cancer, if the treatment has not worked.

I feel like there was a picture specifically for episode 19 of House holding what appeared to be a chemo bag in Wilson's office sometime ago, with the description "House agrees to help Wilson." So I think it is specifically for 19.

I know that there's no way in h*ll that House is going to let Wilson die if there's something he can do about it. So yeah, like you I see him promising Wilson to not to go to the hospital and then taking him anyway (or perhaps telling his team to come to them to treat Wilson?). House loves Wilson way too much to agree to his wishes to die just because the cancer treatment is killing him at that moment.

But I can't get my head around the unplanned vacation/road trip. What is that about then? :/

The great thing that's been mentioned before by others here is that if House does indeed betray Wilson's trust in this way, by saving his life when Wilson wants to either die quickly or cure the thing once and for all, it would be as though House had taken on Stacy's role, back when he had the infarction: going against someone's will to save their lives, because he loves them. Wilson is doing something incredibly Houseian--nearly the same thing House did with his leg, risking death to get a shot at a "best case scenario" even though it was likely going to take his life--and for once, House is the spectator. If House does do this, I imagine there will be some amount of tension between them, to a greater or lesser extent, in the following episodes.

The road trip seems a little bit more amiable than I expected, but think of the title "Post Mortem". It's likely or at least possible the road trip serves in part as time for House and Wilson to go over what happened in the episode prior, as a post mortem can mean many things, one of which is the discussion of something that has come to pass (like what we do here after an episode has aired). It seems likely the episode title has a double meaning, as many House episode titles do.

A lot remains to be seen, and this is just a theory, though.
